Red Hat Enterprise Linux 5 system fails to boot with "switchroot: mount failed" error and LVM physical volumes can't be located

Issue

  • System fails to boot, switchroot error

  • Some changes were made to swap and commented out some things in fstab and removed some PVs.

  • Booting the server into linux rescue mode gives messages “there is no Linux partition found”.

Environment

  • Red Hat Enterprise Linux 5
  • LVM managed root disk
