Hypervisor goes non-operational with iscsid: conn 0 login rejected: initiator failed authorization with target errors

Issue

• RHEV hypervisor goes non-operational when trying to bring it out of maintenance mode

• The following is in /var/log/messages:

Jul 16 18:52:10 localhost iscsid: conn 0 login rejected: initiator failed authorization with target
Jul 16 18:52:10 localhost iscsid: Connection4574:0 to [target: $iqn, portal: $portal] through [iface: default] is shutdown.

• And the following may be present in /var/log/vdsm/vdsm.log:

IscsiAuthenticationError: (24, ['Logging in to [iface: default, target: $iqn, portal: $portal] (multiple)'], ['iscsiadm: Could not login to [iface: default, target: $iqn, portal: $portal].', 'iscsiadm: initiator reported error (24 - iSCSI login failed due to authorization failure)', 'iscsiadm: Could not log into all portals'])
